Vivant Health & Community Resource Project Launch Community Supports Asthma Remediation Program to Support Those Impacted by Conditions
SACRAMENTO, Calif., April 22, 2025 /PRNewswire/ -- With the support of funding and technical assistance from the Department of Health Care Services PATH-CITED Grant Initiative and the PATH-CITED TA Market Place, respectively, Vivant Health and Community Resource Project, Inc. (CRP) have announced a groundbreaking partnership to launch an Asthma Remediation Community Supports (AR-CS) Initiative, aimed at serving individuals and families adversely affected by asthma in the greater Sacramento region.
Vivant Health Launches Community Supports Asthma Remediation ProgramThrough this initiative, Vivant Health will assess patient healthcare needs and connect eligible families with CRP, which will provide in-home assessments, asthma remediation services, and access to additional health-related resources. The program will focus on minority and disadvantaged communities, where multi-family households in older homes often face poor ventilation, mold issues, and outdated heating and A/C systems. Many of these families cannot afford necessary repairs, making the provision of these AR-CS services essential for managing asthma symptoms and triggers, improving living conditions and overall health outcomes.

PATH CITED funding is designed to aid in the delivery/expansion of CalAIM's Enhanced Care Management and Community Supports Initiatives. As a recipient, Vivant Health has partnered with CRP to plan, develop and implement a program to deliver Asthma Remediation Community Supports services to eligible Medi-Cal Managed care members. The goal is provide education, training, and home assessments for 200 families over the next year. The commitment does not end here! Our program start up experience will inform and drive improvements and preparation for upcoming State-planned changes to asthma related benefits/services.
