The Target: BP™ program, a joint initiative of the American Heart Association and the American Medical Association, acknowledges health care providers who demonstrate dedication to following the latest evidence-based guidelines in cardiovascular care. In a remarkable achievement, as a whole, 21 of the 22 nursing homes on the list are part of Centers Health Care, and all of them have achieved either Gold+ or Gold status. The Gold+ recognition, the highest level achievable, demonstrates that these recognized Centers Health Care facilities not only met but exceeded the program's rigorous standards for blood pressure management. This recognition highlights the organization's consistent approach to cardiovascular health management across its network.
